- The Delhi-bound Sikkim Mahananda Express derailed at Bihar's Ara Junction railway station on September 7, and railway records show the loco pilot had not completed the required route learning. The Railway Board had warned all zones about the importance of route learning in a written communication dated August 24, 2026, two weeks before the incident. Following the derailment, the Prayagraj Division suspended the loco pilot, the assistant loco pilot, the deputy crew controller and the chief loco inspector over allegations of deploying crew without proper route learning.
- The board's earlier warning highlighted concerns about crew deployment practices, and the suspensions reflect an effort to enforce compliance with training protocols that are critical for safe train operations.
